We are a 501(c)(3) tax-exempt nonprofit committed to transforming lives across 10 New York counties. Our social care network provides comprehensive support for individuals with disabilities through the New York State Social Care Network program. From initial assessment through navigation, case management, and project completion, we deliver end-to-end high-quality services. Serving Albany, Fulton, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, and Washington counties, we partner with Healthy Alliance to ensure every person receives the personalized attention they deserve.
